JOB SUMMARY
The Clinical Nurse – Case Management is responsible for competently providing safe and effective care to Women and Children, with minimal supervision according to the comprehensive plan of care implemented by the by the Clinical Nurse Leader – Case Management. The Clinical Nurse – Case Management assesses care needs and liaises with the Clinical Nurse Leader – Case Management to provide the needed care.The Clinical Nurse – Case Management is responsible for ensuring that all duties are carried out to the highest possible standard. The Clinical Nurse – Case Management participates in the supervision and teaching of more junior and new members of staff within the unit. The Clinical Nurse – Case Management is professionally accountable to the Chief Nursing Officer.
DEPARTMENT/BRANCH PROFILE
Case Management provides services to complex or high-risk patients receiving care at Sidra. Although case management is often initiated during the acute inpatient care phase, services are also provided in the outpatient settings to connect Sidra specialized clinics with primary care providers in the community. The case management team works collaboratively with the broader health care team to minimize inpatient length of stay by coordinating patient care across the continuum for patients and families with complex care transition needs. Clinical research focused on education, quality improvements and a culture of patient and family safety will be used to inform and guide best practice standards to assure safe, cost-effective and quality outcomes and contribute to Sidra’s reputation as a high reliability organization.
WORK ENVIRONMENT THE ROLE OPERATES IN
- Clinical roles are performed in an acute care hospital care setting.
- Physical effort required may include walking more than is typical due to the distances between hospital units.

KEY ROLE ACCOUNTABILITIES
- Provides total individualized patient care and assumes accountability for assessing, planning, implementing and evaluating care, discharge planning for each patient and assessing for current and potential problems.
- Follows up on patients post discharge to assess if any issues.
- Performs tasks as needed to support the comprehensive plan of care by the Clinical Nurse Leader – Case Management.
- Delivers nursing care to patients and rounds to support efficient patient pathways.
- Maintains Sidra Qatar Resources Database.
- Promotes a safe environment and safe patient care by embracing a safety culture and identifying areas of risk to the Clinical Nurse Leader – Case Management.
- Accurately, concisely and appropriately documents care, interventions and outcomes.
- Contributes to the achievement of clinical quality outcome indicators and patient satisfaction by demonstration of extraordinary service.
- Utilizes a professional practice framework and evidence based care, identifies improvement areas while minimizing risks creating care that is optimal for our patients and families.

- Works in collaboration with Case Management to achieve goals.
- Participates in new staff/student orientation, assists in maintaining a suitable learning environment and acts as a preceptor/mentor in the supervision and teaching of new staff/students to develop their competence.
- Maintains a professional demeanor and upholds the organizational values at all times.
- Acts as a role model for junior staff and assists in promoting and sharing knowledge via mentorship/preceptorship programs in line with relevant research and evidence based practice.
- Follows all Hospital related policies and procedures.
- Performs other related duties as assigned.

- Assists to arrange medically necessary equipment for discharge

QUALIFICATIONS, EXPERIENCE AND SKILLS – SELECTION CRITERIA

- Bachelor Degree in Nursing (4 years)
- Registered Nurse
PREFERRED: Post graduate certificate in specialty if applicable in country of origin

- Eligible to be licensed as a nurse in Qatar
