The Clinical Pharmacist serves as a pharmaceutical caregiver by efficiently designing, recommending, monitoring, and evaluating patient specific pharmacotherapy to ensure optimal outcomes. The Clinical Pharmacist works closely with the Medical and Nursing staff to ensure Patients receive their safe therapeutic treatment informed by best practice.
The post-holder will be familiar with all aspects of a hospital pharmacy service including verification and dispensing of medications used for treatment at Naufar. The Clinical Pharmacist serves as a Pharmacy resource for Pharmacists, Pharmacy Technicians, Medical and Nursing staff, as well as other Healthcare Professionals. The Clinical Pharmacist must comply with all the rules governing the role of a pharmacist as issued by the State of Qatar.
Key Accountabilities :
- Assess, plan, and implement medication assisted treatment.
- Check prescriptions to ensure that there are no errors and that they are appropriate and safe for the individual client.
- Provide advice on the dosage of medicines and the most appropriate form of medication, for example, tablet, injection, ointment or inhaler.
- Prepare and quality-check sterile medications, for example, intravenous medications.
- Ensure medicines are stored appropriately and securely.
- Produce local guidelines for medication use and conduct medication reconciliation on residential patients.
- Manage the formulary, inventory system and procurement.
- Manage the safe disposal of waste medications and illicit substances.
- Attend multidisciplinary case meetings, take client drug histories and participate in decision-making on appropriate treatments.
- Assist in conducting clinical research, adding to the evidence base of practice in relevant area.
- Develop, organize and operate Naufar pharmaceutical services that is aligned with Naufar’s Mission Vision and Strategic plan.
- Review and interpret physician’s drug orders for clarity, rationality, dosage form, allergies, interactions, incompatibility, etc. and make recommendations to prescribers if changes are required.
- Maintain control and security of all drugs including dispensing, record keeping and secured storage of narcotics and controlled medications.
- Provide medication counseling to patients and families.
- Check medications prepared by Pharmacy Technicians for accuracy prior to dispensing.
- Assist in supporting Pharmacy preceptorship with students and external trainees.
- Document procurement, distribution and dispensing of medications in an accurate manner.
- Liaise with other Healthcare Professionals to ensure the delivery of safe, and effective treatments.
- Follow all related Naufar policies and procedures.
- Monitor drug usage and report to the Director of Pharmacy.
- Act as primary contact point for Naufar staff in relation to Pharmacy issues.
- Ensure medications are stored securely and appropriately.
- Promote and maintain quality in all services and ensure continuous quality improvement measures are in place.
- Carry out Pharmacy controlled drug checks.
- Assist in the development and implementation of policies and procedures.
- Document all interventions and medication errors.
- Attend any assigned multidisciplinary case meetings.
- Update current skills and knowledge in relevant practice areas as required.
- Participate in any internal or external courses / meetings as appropriate.
- Perform other related duties or assignments as directed.
Job Requirements :
Professional / Academic Qualifications :
Accredited Master’s degree or Doctor of Pharmacy.Licensed as a clinical pharmacist in home country.Up-to-date CPD in relevant areas.Postgraduate qualification in a relevant subject area.Experience :
